If you currently have COVID-19, you will must complete the full 10-day isolation period before coming to our facilities to receive a COVID-19 vaccination. If you have an urgent medical need, please notify your provider you are COVID-19 positive before your appointment.
Have you ever had a severe allergic reaction to a component of the COVID-19 vaccine, including polyethylene glycol (PEG - which is found in some medications, such as laxatives and preparations for colonoscopy procedures), polysorbate, or a previous dose of COVID-19 vaccine, which caused hives, swelling, shortness of breath, wheezing, or required treatment with epinephrine or EpiPen®, or that caused you to go to the hospital?
Based on your history of allergic reactions, it is recommended that you do not receive the COVID-19 vaccination. Please contact your primary care provider if you have additional questions.
