If you did not upload the patient's medical records directly to your web form's submission, then please send the patient's medical records at your earliest convenience.
Options for sending medical records to us are listed below.
Please print our cover sheet, fill in the patient's name, and attach it to the pertinent medical records. Please fax the records to our Health Information Management Department.

Epic users can view patient information from UI Health Care using Care Everywhere. Information such as client summaries, encounter summaries, lab results, and progress notes.
If the patient's medical records are already uploaded to EPIC and you are a Care Everywhere user, then there are no additional steps.
Images can be pushed via LifeImage or sent on a disc in DiCom format and sent via Fedex.
