Do you need to schedule or reschedule your appointment? Call, submit requests online, or use MyChart to manage your care.
Call 515-282-2921 and ask for your doctor’s scheduler to schedule, reschedule, or ask questions about an upcoming appointment appointment.
New patients can request care from our request an appointment form online.
For new patients that need to reschedule, please call 515-282-2921 and let the receptionist know that you need to cancel your initial visit. You will then be transferred to a new patient referral specialist for rescheduling.
