Continuity of Care
The Continuity of Care program is a family-centered service of University of Iowa Stead Family Children's Hospital, helping families with children who have special health care needs. We offer support during your child's stay, clinic visits, and after you go home.

Continuity of Care coordinators help identify and obtain services for patients and families during your child's hospital stay, clinic visits, and when they return home. We can also:
- Coordinate care
- Assess your child's and family's needs
- Find answers to your questions and concerns about your child
- Enhance communication between health professionals
- Assure your child and family receive community services needed
- Provide physical and emotional support
- Assist your family to be a strong voice for your child
- Ensure your family feels ready for discharge
After your child leaves the hospital, care coordinators can be your liaison between hospital and community providers, including home nursing agencies, primary care physicians, and other community programs.
