Important things to remember about warfarin

  • Take your warfarin exactly as you are told.
  • Get your blood tested as directed.
  • Check with your health-care provider before you change, start, or stop taking any medicines or herbal products.
  • Do not make big changes in the food you eat, lifestyle, or activities without talking to your health-care provider first.
  • Limit or stop drinking alcohol while on warfarin.
  • Do not take two doses if you miss a dose. Record the day you missed your dose.
  • Tell anyone giving you medical or dental care that you are taking warfarin.
    •  Certain procedures and surgeries cannot be done if you are on warfarin.
  • Carry a patient ID card.
  • You may wear a medical alert bracelet or necklace.

Call your health-care provider right away if you have any of the following:

  • Unusual bleeding or bruising
  • You get sick or injured
  • Signs of a blood clot
  • If you are planning or become pregnant
  • You miss doses for two or more days
