Methods to access your medical records
If you would like access to your medical record, please talk to your health care team.
You may also sign up to view your child’s or another adult’s medical records through MyChart by visiting the MyChart Forms website and completing the appropriate application form.
The preferred method to receive medical record requests is in writing or via MyChart. However, when these options are not available, requests may be sent via e-mail or fax.

You can request a pre-printed form be sent to you via e-mail or by mail, please e-mail HIM-ConsentForm@uiowa.edu.
Completing a form on behalf of a child?
To avoid delays in processing, please review the form carefully and complete all sections. If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign.

Ordering a Birth Certificate
Birth certificates for all individuals born at UI Hospitals & Clinics are kept on file at the Johnson County Recorder's Office. Requests for official copies of birth certificates should be addressed to:Recorder's Office
