First Dose (Primary Series)
UI Health Care primarily offers the Pfizer-BioNTech COVID-19 vaccine but has limited availability of Moderna.
You can discuss manufacturer selection with your clinic at the point of vaccination.
Parental or guardian consent is required for a vaccination appointment for those under the age of 18. A parent or guardian must accompany the minor to their vaccination appointment to provide consent. If the minor is not accompanied by a parent or guardian, we will be unable to provide the vaccination.

Second Dose (Primary Series)

Johnson & Johnson (Janssen)
UI Health Care is not currently offering doses of the Johnson & Johnson (Janssen) COVID-19 vaccine.
If you have previously received the Johnson & Johnson (Janssen) COVID-19 vaccine and would like to schedule a second (booster) dose, please contact a local pharmacy.
Third Dose (Primary Series)
Only children ages 6 months to 4 years old who received Pfizer-BioNTech as their first and second dose need a third dose in their primary series.

Please make sure to select a date that is at least 8 weeks after your child received their second dose of the Pfizer-BioNTech COVID-19 vaccine.

Additional Dose (for those who are moderately to severely immunocompromised)
People are considered moderately to severely immunocompromised if:
- Been receiving active cancer treatment for tumors, or cancers of the blood
- Received an organ transplant and are taking medicine to suppress the immune system
- Received a stem cell transplant within the last 2 years or are taking medicine to suppress the immune system
- Moderate or severe primary immunodeficiency (such as DiGeorge syndrome, Wiskott-Aldrich syndrome)
- Advanced or untreated HIV infection
- Active treatment with high-dose corticosteroids or other drugs that may suppress your immune response

Pfizer-BioNTech
Children are eligible for an additional dose if it has been 28 days after you’re their second dose of the Pfizer-BioNTech vaccine.
AND
They are moderately to severely immunocompromised.

Booster Dose
The Centers for Disease Control and Prevention (CDC) recently authorized updated booster doses of the Moderna and Pfizer-BioNTech COVID-19 vaccines designed to protect against the omicron variants.
