Do you currently have any symptoms of COVID-19 (fever, chills, sore throat, new or worsening cough or shortness of breath, loss of taste or smell)?

Because you are feeling sick today, we recommend waiting until you feel better to receive the COVID-19 vaccine. Have you ever had a positive test for COVID-19? Or has a doctor ever told you that you had COVID-19?

If you currently have COVID-19, you will must complete the full 10-day isolation period before coming to our facilities to receive a COVID-19 vaccination. If you have an urgent medical need, please notify your provider you are COVID-19 positive before your appointment.

Have you ever had a severe allergic reaction to a component of the COVID-19 vaccine, including polyethylene glycol (PEG - which is found in some medications, such as laxatives and preparations for colonoscopy procedures), polysorbate, or a previous dose of COVID-19 vaccine, which caused hives, swelling, shortness of breath, wheezing, or required treatment with epinephrine or EpiPen®, or that caused you to go to the hospital?

Based on your history of allergic reactions, it is recommended that you do not receive the COVID-19 vaccination. Please contact your primary care provider if you have additional questions.
