Find-A-Provider profiles on uihc.org are limited to physicians, physician assistants, nurse practitioners, social workers, nurse navigators, psychologists, rehabilitation therapists, pharmacists, and other full-time staff members that patients can make an appointment with or be referred to for an appointment. 

If you are a provider who fits these criteria and do not currently have a provider profile on uihc.org, you may complete the New Provider Profiles Form.
