Men's Health Assessment
The UI Health Care Men's Health Clinic invites you to take two to three minutes to complete a men’s health questionnaire. This set of questions was developed by the University of Iowa for personal use. You won't be asked to identify yourself, and only you will see the results. The University of Iowa will not have access to your results or have any way of knowing how you responded to the questions.
Our men's health questionnaire will guide you through 20 general questions that address your weight, heart health, mental health, sleep health, and urological health.
Results from this assessment can be shared with your own physician, along with any health concerns you may have. Feedback from this wellness assessment is not meant to replace your own physician’s advice.
