Diabetes is a condition in which the body does not properly process food for energy, resulting in high blood sugar. Gestational diabetes is diabetes that starts or is first diagnosed during pregnancy. High blood glucose levels over long periods of time have been shown to result in blood vessels being damaged. Damaged vessels can lead to organs not functioning correctly.
Advanced Technology
Patients eligible for this program will be given a new cellular-enabled blood glucometer. This advanced device works like a regular glucometer to test your blood sugar levels. It also uses a secure network to send glucose readings directly to our care team.
Coordinated Care
Members of our care team have specialized training in diabetes education and glucose monitoring. They work together to identify concerns and set goals that will improve the health of you and your baby.
