Our bariatric surgery program on our university campus is unique in its approach to the care of the morbidly obese patient. Our multi-disciplinary team approach helps our patients achieve lasting weight-loss that:
- Reduces life-threatening risk factors

Bariatric Surgery Services at the University Campus
The two most-commonly performed bariatric surgeries in our program are the Laparoscopic Roux-en-Y Gastric Bypass (LRYGB) and Laparoscopic Sleeve Gastrectomy (LSG).

Qualifications for Bariatric Surgery
Our program honors both the long‑standing and the updated evidence based national guidelines for bariatric surgery. This means we consider candidates who:
- Are at least 18 years of age
Meet the traditional and updated Body Mass Index [BMI] criteria based on insurance requirement:
- BMI ≥ 40 with or without a serious obesity related condition
- BMI ≥ 35 with or without a serious obesity related condition
- BMI ≥ 30 with type 2 diabetes
STEP 1: Find out if you are a candidate for bariatric surgery.
Complete and submit our online bariatric surgery questionnaire.
Note: The form cannot be saved once started, so you will need to complete it in a single session.
STEP 2: Once you have submitted your questionnaire, please allow us time to thoroughly review your information. This screening process allows us time to carefully review your insurance benefits and health information. If you do not hear from us after one week, please contact our office.
Not every person with a diagnosis of obesity is eligible for bariatric surgery. Patients must meet specific qualifications.
Bariatric Surgery could be a solution if you feel ready to commit to the following:
- A consistent exercise program
- Counseling and follow-up care
- Long-term dietary changes that include vitamin and mineral supplements
