Social workers in the Health Care Benefits Assistance Program (HCBAP) help patients and families apply for medical assistance programs that may assist with their health care related costs/needs:
Providing information and assistance in completing applications for such programs as: Medicaid, HCBS Waivers, Social Security Disability, and Supplemental Security Income (SSI)
Assisting with, clarifying, and coordinating benefits through Social Security Administration, Disability Determination Services, and the Department of Human Services
