Care Coordination Division/Social Services provides services to patients, and their families, which enable them to make the best use of health care resources.
The department works to reduce social, financial, and medical barriers in order to optimize patient and family functioning. Our professional staff members also are committed to teaching and research activities designed to train the next generation of health care providers.
Our mission is to provide direct care including assessment, treatment, interventions and recommendations to facilitate movement through the continuum of care to patients and their families. We do this by:
Serving patients/families, throughout the health care continuum, who are vulnerable and who need assistance with financial resources, utilizing community resources, or with personal/family issues;
Providing interdisciplinary leadership and promoting collaboration to continually improve discharge planning and continuity of care services;
Fostering partnerships with community health and human service providers;
Advocating for health care policy and legislation that recognizes and accommodates the need for affordable, accessible, and patient-centered health care;
Training the next generation of social work clinicians.
Areas of Care Coordination Division/Social Services
Clinical social workers provide a dynamic assessment of a patient’s health status, personal reactions to illness or injury, and the patient’s and family’s capacity to manage these events.
Social workers in the Health Care Benefits Assistance Program (HCBAP) help patients and families apply for medical assistance programs that may assist with their health care related costs/needs.
UI Health Care social workers work with qualified patients and their families to acquire medications from pharmaceutical companies at little or no cost.
