Care Coordination Division/Social Services provides services to patients, and their families, which enable them to make the best use of health care resources.
The department works to reduce social, financial, and medical barriers in order to optimize patient and family functioning. Our professional staff members also are committed to teaching and research activities designed to train the next generation of health care providers.
Our mission is to provide direct care including assessment, treatment, interventions and recommendations to facilitate movement through the continuum of care to patients and their families. We do this by:
Serving patients/families, throughout the health care continuum, who are vulnerable and who need assistance with financial resources, utilizing community resources, or with personal/family issues;
Providing interdisciplinary leadership and promoting collaboration to continually improve discharge planning and continuity of care services;
Fostering partnerships with community health and human service providers;
Advocating for health care policy and legislation that recognizes and accommodates the need for affordable, accessible, and patient-centered health care;
Training the next generation of social work clinicians.
