To request new artwork for an area that serves patients, or to have artwork moved or removed, please complete the
Online Project Art Request Form
No work will be done by the Project Art staff until the form is completed and received. Due to the overwhelming number of requests we receive, please allow a minimum of 2-4 weeks for requests to be filled.
