How to access your medical records
You may also sign up to view your child’s or another adult’s medical records through MyChart by visiting the MyChart Forms website and completing the appropriate application form.
Please note, medical records from Medical Center Downtown (Formerly Mercy Iowa City) prior to May 4, 2024 will not yet be available in MyChart. With such a large number of patient records to transfer, this will need to happen over a much longer period of time.
The preferred method to receive medical record requests is in writing or via MyChart. However, when these options are not available, requests may be sent via e-mail or fax.
Use our HIPAA-compliant form to authorize the release of medical information.
You can submit the completed form by email, fax, or direct mail.

You can request a pre-printed form be sent to you via e-mail or by mail, please e-mail [email protected].
To avoid delays in processing, please review the form carefully and complete all sections. If the patient is a minor (less than 18 years of age), a parent or legal guardian must sign.
Questions regarding release of information or the pre-printed form, may be directed to the Release of Information Office at 1-319-356-1719, Monday through Friday between 8 a.m. to 5 p.m.

Ordering a Birth Certificate
Birth certificates for individuals born at UI Health Care Medical Center on University Campus or Downtown Campus (formerly Mercy Iowa City) are available from the Johnson County Recorder's Office and/or the Iowa Department of Health and Human Services.
